Job Description
Role Summary
To provide the highest level of clinical care to all patients paying attention to detail and ensuring a person centred and holistic approach to the individual care is delivered.
To provide leadership to the care team to facilitate the provision of expert, high quality specialist nursing care, advice and psycho-social support.
To act as an expert resource as part of the team on care, both on supportive treatment and end of life care.
To facilitate the ongoing development of nursing and care staff through acting as a role model and resource.
To assess, monitor and respond to changing health needs and report in a timely fashion to senior staff and the GP
To provide day to day operational nursing care, ensuring delivery of a patient focussed service.
To ensure accurate and contemporaneous records are kept, maintaining professional standards and ensuring patient safety is maximised
Principal Responsibilities / Clinical and Professional
To work clinically as a nurse in all areas of care and oversee the planning, implementation and evaluation of care to address patients complex and changing health care needs. The care team works 7 days a week and there is likely to be a requirement for weekend and night working.
To provide advice to support staff and patients especially when patients have end of life care needs ensuring all medical and nursing needs are understood and carried out in accordance with Royal Marsden, QCS policy & care plans.
To liaise with multi-professional teams to improve the way in which nursing and end of life care is provided; negotiating with medical team(s) on an agreed patient care pathway and including ongoing referral to other services as appropriate to ensure a seamless service.
To work in partnership with service users and act as patients advocate and counsel when informed discussion may lead to choices being made concerning treatment options, quality of life issues or preferred place of death.
To extend the scope of practice by undertaking additional training and complying with local policies on maintaining competence, e.g. non-medical prescribing, physical assessment, psychological support.
Strategic and Business Planning
To assist the nursing team to provide high quality nursing and end of life services across the home and ensure that care is provided in line with the latest national and local guidance on clinical standards in collaboration with the GP and any medical consultant involved.
To help ensure that policy decisions are implemented consistently by actively engaging staff.
To provide patient centred care to those requiring end of life care as well as support and advice for those with life limiting/long term conditions who are admitted to the home.
To promote excellence in care and end of life care by maintaining clinical skills through ongoing education.
To identify areas for service development and report to the Director of Nursing & Clinical Development. To be involved in the development and implementation of policies, procedures and guidelines to support changes in practice.
To have an ongoing commitment to the improvement of clinical standards and the development of patient centred strategies in accordance with national guidelines.
Communication
To provide timely communication with patients and professionals about management plans, including a collaborative approach between the sectors involved in patients with palliative and end of life care needs this includes community and hospice teams. Agreeing ceilings of care where appropriate.
To communicate between colleagues in all disciplines actively and roles in order to improve patient safety and experience.
To mentor colleagues and students in clinical and communication skills.
To provide support and development of advance care planning for patients to communicate their wishes for care.
Management and audit
To be an effective Leader and line manager to the Nursing Associates, Nursing Assistants, and others within the team, providing effective clinical leadership to support and inspire colleagues.
To take a role in the recruitment, selection and retention of staff within the Care Team when required.
To ensure the annual appraisal of all staff within the team and undertake Individual Performance Reviews, setting of objectives and personal development plans for individual team member.
To investigate specific incidents or complaints relating to patient care as requested by the Director of Nursing & Clinical Development or Registered Manager and to inform the focus of practice development activities and assist in distributing learning from this.
To lead team meetings and Multi Disciplinary Teams and participate in the formulation of individualised patient care plans and advise and support staff in this decision-making process.
Service Development and Improvement
To facilitate the development of mechanisms for monitoring throughput and uptake of the service as appropriate.
To inform the Registered Manager and Director of Care & Clinical Development regarding practice issues, delivery of service and clinical governance in nursing care.
To take an active role in the peer review process.
Education and Training and Development
To promote a culture and environment which facilitates lifelong learning within the multi-professional team.
To act as a professional supervisor for new team members and training posts within the team and utilise clinical supervision and reflective practice skills to enhance development of self and colleagues in the team to optimise care.
To manage the initiation, development, implementation and evaluation of care educational programmes in response to identified nursing staff
To be accountable for the organisation and provision of education and teaching for patients and their families/carers with care needs and provide access to appropriate resources in conjunction with the local education groups.
Research and Audit
To participate in the development of mechanisms to involve users in practice and service development in care and to ensure their views are incorporated into and influence the future service development.
To develop and implement evidence based practice in care by participating in monitoring and auditing standards of care, measuring outcomes and adjusting policies and competencies accordingly.
When required, to lead on the collection and recording of relevant data required for clinical audit, statistical analysis for research projects and national registers as appropriate.
To ensure that there are comprehensive audit and quality monitoring systems in place to reduce risk and improve standards of care and that lessons are used to inform and improve practices.
To participate in research and development initiatives as appropriate.
Governance
To take responsibility for your own professional development through the appraisal system and work towards completion of an agreed personal development plan, including maintaining NMC registration.
To update your own professional knowledge to promote excellence in clinical practice and to meet the ongoing changing needs of the service. Attend and present at national and international conferences and educational events as appropriate.
To ensure membership of national professional groups to facilitate updating and development within Adult Nursing.
Financial
To exercise effective financial management of the team through the efficient use of manpower.
To manage the team within the allocated budget and to operate with due regard to activity levels.
To provide a specialist palliative care service in line with current national and palliative care guidance.

Health and Safety
In accordance with the Health and Safety at Work Act 1974 and other supplementary legislation, you are required to take reasonable care to avoid injury during work and cooperate with the Home and others in meeting statutory regulations. You are also required to attend statutory training as required to fulfil your duties.

To use in a proper safe manner the equipment and facilities provided.
To refrain from misuse of, or interference with, anything provided in the interest of health and safety and any action, which might endanger yourself and others.
To report as soon as practical any hazards and defects to the Director of Clinical Nursing.
To report, as soon as practical, accidents and untoward incidents and to ensure that accident forms are completed.
Infection Control
All employees of the Home are required to contribute to the management of infection control and be proactive in awareness raising and prevention.
