[Employer hidden — sign up to reveal] is currently recruiting for a Band 6 Community Psychiatric Nurse (CPN) to join the Primary Care Mental Health Service (PCMHS) in St Ives. This opportunity is suited to an experienced mental health professional with a strong background in community and primary care services.

The successful candidate will support adults with mental health needs through assessments, care planning, and ongoing therapeutic support while working closely with multidisciplinary teams and primary care providers.

Key Responsibilities

• Conducting mental health assessments within primary care settings
• Supporting service users with recovery-focused interventions
• Managing caseloads and coordinating ongoing care
• Developing and reviewing care plans
• Liaising with GPs, community teams, and external agencies
• Maintaining accurate clinical documentation
• Escalating safeguarding and clinical concerns appropriately

Requirements

• Valid NMC registration as an RMN
• Previous experience working as a CPN or Mental Health Practitioner
• Community mental health and primary care experience preferred
• Strong communication and case management skills
• Ability to work independently within a multidisciplinary environment
• NHS experience highly desirable
