Community Clinical Nurse Specialist – Band 6 CPN

Key Responsibilities
- Completing specialist mental health assessments for children and young people
- Managing and coordinating care plans within community settings
- Providing therapeutic and recovery-focused interventions
- Working collaboratively with families, schools, and multidisciplinary teams
- Maintaining accurate clinical documentation and risk assessments
- Supporting safeguarding processes and escalating concerns appropriately
- Contributing to service delivery and clinical reviews
Requirements
- Valid NMC registration as an RMN
- Previous experience within CYPMHS or CAMHS services
- Strong community mental health and care coordination experience
- Excellent communication and risk assessment skills
- Ability to manage caseloads independently
- NHS experience preferred
