Care Coordinator – Early Intervention Team for Psychosis | Band 6 CPN

This role involves supporting service users aged 14 to 65 experiencing first episode psychosis or related mental health challenges within a community setting. Key Responsibilities
- Managing and coordinating care for service users within the Early Intervention Team
- Completing mental health and risk assessments
- Developing and reviewing person-centred care plans

Requirements
- Valid NMC registration as an RMN
- Previous experience working as a CPN or Care Coordinator
- Experience within psychosis or early intervention mental health services preferred
- Strong community mental health and caseload management skills
- Excellent communication and organisational abilities
- NHS experience highly desirable
