Successful candidates will support service users through assessments, care planning, risk management, and recovery-focused interventions while working collaboratively within multidisciplinary teams.

Key Responsibilities

• Completing mental health and risk assessments
• Managing caseloads and coordinating ongoing care
• Developing and reviewing person-centred care plans
• Delivering community-based mental health interventions
• Working closely with multidisciplinary teams and external services
• Maintaining accurate clinical documentation
• Escalating safeguarding and clinical concerns appropriately

Requirements

• Valid NMC registration as an RMN
• Previous experience working as a CPN within community mental health services
• Strong assessment and care coordination skills
• Ability to manage caseloads independently
• Excellent communication and organisational abilities
• NHS experience preferred
