Summary
The Community Matron will support patients of all ages with diverse and often complex health needs across the Liskeard community. They will provide personalised, holistic care for adults with long-term conditions, frailty, and multiple comorbidities — helping patients live well at home, in care homes, or in other community settings. Given that a significant proportion of our population is older, this role will include a focus on supporting frail and elderly patients, while also addressing the broader needs of adults of working age with complex or chronic conditions.
Key Responsibilities
- Provide advanced clinical assessment, diagnosis, and management of adults with complex, long-term conditions — including frail, elderly, and housebound patients — in their homes, care homes, and community healthcare settings.
- Deliver proactive clinical care, including falls risk assessment, polypharmacy review, and anticipatory care planning.
- Work closely with families, carers, and social care teams to support patients with complex needs.
- Lead and coordinate clinical care pathways with GPs, community nursing, care homes, therapy teams, social care, and voluntary sector partners.
- Support the development of proactive care initiatives and early intervention within the practice and PCN.
Qualifications and Experience
- Registered Nurse with valid NMC registration
- Independent Nurse Prescriber (V300)
- Significant post-registration experience in community or primary care, managing complex caseloads
- Experience of multi-agency, multidisciplinary working
- Knowledge of current NHS policy, community nursing models, and primary care priorities
- System used: EMIS
