Job Summary

Join our patient-focused GP practice as a Care Navigator and make a real difference every day. In this vital frontline role, you'll be the friendly first point of contact for patients, helping them access the right care quickly and efficiently whether that's a GP, nurse, pharmacist, or local support service. No two days are the same, and your ability to listen, guide, and reassure will have a direct impact on patient experience and outcomes.

Main Duties of the Job

  • Act as the first point of contact for patients, managing enquiries both by phone and face-to-face
  • Assess patient needs and signpost to the most appropriate clinician or service
  • Book, amend, and manage appointments efficiently following practice protocols
  • Prescription requests
  • Provide information on healthcare services and support self-care where appropriate
  • Prioritise urgent requests and escalate concerns safely
  • Maintain accurate patient records using clinical systems
  • Work closely with the wider practice team to ensure smooth patient flow and high-quality care delivery
  • Act as chaperone

Job Responsibilities

  • Greet patients and visitors in a professional and friendly manner
  • Answer incoming telephone calls and manage patient enquiries
  • Book, amend, and cancel appointments in line with practice procedures
  • Direct patients to the appropriate service or clinician as needed
  • Manage incoming and outgoing correspondence, including emails and letters
  • Maintain accurate patient records on clinical systems
  • Handle confidential information in accordance with GDPR and practice policies
  • Support administrative tasks such as scanning, filing, and processing documents
  • Ensure the reception area remains organised and welcoming

Person Specification

Qualifications (Desirable)

  • Previous experience working within a GP practice, primary care, or healthcare setting
  • Experience using clinical systems such as EMIS, SystmOne, or similar
  • Knowledge of NHS services, pathways, and local community support services
  • Experience of care navigation, signposting, or patient triage
  • Understanding of safeguarding procedures and when to escalate concerns
  • Relevant training in Care Navigation, Active Signposting, or Patient Services
  • Healthcare or customer service qualification (e.g. NVQ Level 2 or equivalent)
  • Experience working in a fast-paced environment with high volumes of patient contact
