FULL DESCRIPTION

Diabetes Specialist Nurse

Cross site role between Hospital of St Cross and University Hospital Coventry. Provide advance, specialist nursing care for people with diabetes across two clinical sites with a particular focus on insulin pump therapy. The post holder will be part of a larger team responsible for education, complex case management, service development and ensuring high quality, evidence based care with improved patient outcomes. The role will be largely outpatient work, with support for in patients who attend on insulin pump therapy.

Main Duties

  • Provide specialist care and management for patients with Diabetes.
  • Assess, Plan, Implement and Evaluate diabetes treatment.
  • Educate patients on self management, lifestyle and technologies.
  • Provide training and advice to staff and contribute to service improvement.

Job Responsibilities

  • Deliver expert care for patients with Type 1 and complex Type 2 diabetes, including those using insulin pump therapy.
  • Initiate, adjust, and optimise CSII and multiple daily injection insulin regimens.
  • Education around management of acute diabetes presentations including Diabetic Ketoacidosis and severe hypoglycaemia.
  • Interpret glucose data (CGMS, flash and capillary readings) to guide treatment decisions.
  • Deliver structured education to patients on insulin pump use, sick day rules, carbohydrate counting.
  • Assist in providing training and support for healthcare professionals in diabetes technology.
  • Work closely with consultants, nurses, and dietitians to commence patients on insulin pump therapy and provide follow up reviews.
  • Work with insulin pump providers to ensure safe use and support patients with new technologies.
  • Support other team members with use of insulin pump therapy.
  • Assist in service coordination of insulin pump clinics across two sites.
  • Maintain accurate clinical documentation and ensure safe practice.
  • Participate in incident reporting, investigations, audits, and shared learning.
