Senior Community Nurse

Job summary
Are you ready to take the next step in your Community Nursing career? We're offering an exciting opportunity for a Senior Community Nurse in our North and West Bristol Locality. This role is perfect if you are a Community Nurse looking to expand upon your knowledge and skills and step into a senior position. You'll gain valuable leadership experience, enhance your clinical knowledge, and contribute to the delivery of high-quality, patient-centred care in the community. We can offer full time or part time positions (minimum of 30 hours per week to meet service needs). You will be fully supported to develop your leadership and management skills, gain experience in a senior nurse role, work within a supportive and innovative team, and make a meaningful impact in local communities.
Main duties of the job
- Complete holistic assessments of service users.
- Develop and deliver person centered care plans.
- Line manage junior staff and contribute to their development through supervision, 1:1s and training.
- Undertake and complete the appropriate training, supported by a robust competency framework.
- Communicate with the senior leadership team to ensure smooth delivery of service.
- Identify risks and act accordingly to minimise risk.
- Coordinate shifts and delegate/allocate care to the team.
- Act as a point of escalation for staff.
- Undertake further training as required to develop extended skills.

Person Specification
Qualifications
Essential: BSc or equivalent experience in Adult Nursing, Current NMC registration, Evidence of continuing professional development.
Skills, knowledge and experience
Essential: Able to effectively lead a team, make clinically reasoned judgements, supervise staff and students, manage time and caseloads. Desirable: Recent community experience including management of frail complex patients.
