Inclusion Health Senior Nurse

- Salary: £55,524 - £62,652 per annum
- Contract: Fixed term (19 months)
- Working pattern: Full-time

Job Summary
We have an exciting opportunity for an experienced nurse with an interest in Homeless Health to join our specialist Homeless Pathway Team here at [Employer hidden — sign up to reveal] for a 7-month secondment. You will be part of an established and supportive team, working with people experiencing homelessness within the hospital to improve their health outcomes, patient experience, and to work alongside our partners to support the flow of discharge into the community.
Main Duties of the Job
- Use expert clinical skills in assessing and creating treatment plans
- Refer and signpost to relevant clinical and non-clinical areas, liaising with health, social, housing, and specialist partners
- Manage a caseload and use active listening to engage and advocate for patients
- Be a role model, teaching and advising other staff and partners
- Lead the team, oversee caseload, provide expert advice, and offer regular supervision and training
- Maintain timely and accurate documentation and data entry
- Lead on delivery of specific service targets (e.g., GP registration), attend multi-agency meetings, and assist with reports and project work

Person Specification - Essential
- DBS certificate
- Qualified Nurse
- Good knowledge regarding local support options for people experiencing homelessness
- Extensive clinical experience in homelessness
- Good clinical knowledge in inclusion health
- Extensive experience in discharge planning
- Extensive experience of acute hospital setting
