Senior Social Worker

Job Summary
As a Senior Social Worker, you will work as part of a multidisciplinary team to support people with Acquired Brain Injuries (ABI), progressive neurological conditions, Autism Spectrum Conditions (ASC), and Deaf Mental Health problems. You will manage a complex caseload, conduct social care assessments and risk assessments, and complete pre-admission for discharging service users. You will support and guide junior team members.
Main Duties of the Job
- Liaising with home authorities (Health, SSD, Probation) at admission and discharge stage
- Collaborating with home authorities, service users, and families in decision making and care planning
- Maintaining electronic/integrated records and completing administrative procedures
- Preparing reports for Mental Health Review Tribunals, appeals to MH Act Independent Hospital Managers, and attending hearings
- Facilitating access to welfare rights, housing, advocacy services, and associated support services
Requirements
- CQSW or CSS or DipSW or Degree in Social Work
- Relevant experience in a similar setting
- To fully engage in Breakaway training and MVA training
