Practice Nurse

This is an exciting opportunity for a highly motivated Practice Nurse to join our multi-disciplinary practice team at [Employer hidden — sign up to reveal].

  • Salary: £21.47 to £23.23 an hour
  • Contract: Permanent, Full-time
  • Location: Lion Street, Oakengates, Telford, TF2 6AQ

Main duties of the job

  • Practice Nurse preferably with experience working in a General Practice setting
  • Experience in managing diabetes, asthma and COPD is desirable but not essential
  • Looking to work full time and with a desire to work as part of a supportive and enthusiastic team
  • To include work on one Saturday every two months and one weekday evening every two weeks (Extended Access cover)
  • Desirable to have own transport and ability to drive to be able to offer support and care to housebound patients

Job responsibilities

  • Adhere to NMC revalidation requirements and practice within the NMC Code of Conduct
  • Comply with national infection control standards and health and safety guidelines
  • Consult with patients and their families to undertake assessments of patient need
  • Support in the long-term condition management within the organisation
  • Develop, implement and embed an effective care management programme for the frail and housebound patients
  • Support public health and screening programmes, including immunisations, vaccinations, and cervical screening
  • Provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
  • Support new-to-practice nurses and other healthcare professionals
  • Teach and advise patients and their families on how to manage their condition
  • Participate in clinical audits and research projects
  • Work within national and local protocols
  • Recognise boundaries of practice and refer appropriately
  • Support in delivery of enhanced services
  • Be aware of duties regarding Safeguarding Adults and Children
  • Manage patient complaints and identify learning from incidents
  • Undertake mandatory training
  • Contribute to clinical governance
  • Attend annual appraisal
  • Provide immunisations including influenza, pneumococcal, shingles, RSV, infant immunisations
  • Provide travel advice and immunisation services
  • Perform cervical smears, pregnancy tests, contraceptive advice
  • Assist GPs with minor surgeries and coil fittings
  • Carry out ECGs, venepuncture, urinalysis, wound care
  • Maintain accurate clinical records using SNOMED CT codes
  • Contribute to QOF targets
  • Maintain chronic disease registers
  • Perform practice housekeeping duties
  • Monitor and order medical stock
  • Chaperone patients
  • Liaise with external services for vulnerable patients
  • Attend practice meetings
  • Conduct home visits as required

Experience Essential

  • Good communication skills
  • Good ICT knowledge and skills
  • Organisational skills and time management
  • Professional, calm, efficient manner
  • Self-motivated, works well independently and in a team
  • Works safely autonomously and with other professionals

Experience Desirable

  • Previous primary care experience in GP practice nursing team
  • Experience of EMIS and Docman
  • Experience of clinical audit and research
  • Understanding of national and local guidelines
  • Understanding clinical governance and risk management

Qualifications Essential

  • Registered with NMC and maintains revalidation
  • Working at Registered Nurse Level Practice
  • Enhanced DBS check

Qualifications Desirable

  • Level 6 diploma or postgraduate certification in long-term conditions care/public health
  • Evidence of continuing professional development
  • Valid UK driving licence
