Job Summary
Are you an enthusiastic and dedicated Advanced Clinical Practitioner? An exciting opportunity has arisen for an Advanced Clinical Practitioner to join the Sefton Acute Visiting Service (AVS) on a bank basis. The AVS responds to both own home and care homes, operating Monday to Friday 09:00-18:00 and community response Monday to Sunday 08:00-20:00. The successful candidate will hold an MSc in Advanced Clinical Practice or Credentialing and a Non-Medical Prescribing qualification.

Job Responsibilities
Clinical Practice: Assess and examine patients, plan and evaluate management, provide medication advice, prioritise emergencies, triage referrals.
Communication: Use sensitive communication styles, maintain effective communication with patients and stakeholders.
Non-Medical Prescribing: Prescribe within scope and comply with [Employer hidden — sign up to reveal] NMP governance processes.
Delivering a Quality Service: Work within competence, maintain records, deliver evidence-based care.
Leadership and People Development: Take responsibility for learning, support staff development, lead change.
Corporate Responsibility: Work as a team member, delegate appropriately.
Management of Risk: Assess and manage risk, ensure safety.
Managing Information: Use technology for planning and monitoring.
Learning and Development: Undertake mentorship, assess own learning needs.
Person Specification
Skills & Attributes (Essential): Advanced clinical practice, clinical examination, leadership, communication, negotiation, change management, motivation, flexibility, team player.
Qualifications (Essential): Registered healthcare professional with MSc Advanced Clinical Practice or credentialing, V300 Non-Medical Prescriber.
Experience (Essential): Senior clinician, autonomous practice, quality improvement, primary and urgent care.
