[Employer hidden — sign up to reveal] is hiring a Community Registered Nurse for the West Hub at Acomb Health Centre, York. This is a permanent part-time Band 5 role with a salary of £32,073 to £39,043 a year pro rata. The closing date is 21 May 2026.
Job Summary
Are you a Registered Nurse looking for a new challenge, working autonomously within Community Nursing? We have teams based in the following areas:
- West Hub - Acomb Health Centre
- North Hub - Haxby Health Centre
- Selby/South Hub - Selby Hospital, Tadcaster Health Centre or Sherburn in Elmet
- East Hub - Tang Hall Clinic
- South Hambleton and North Ryedale - Easingwold, Pickering and Kirkbymoorside
*Occasionally staff will be required to work in a hub that differs from their base.*
We currently have a vacancy in the West Team. We are community nursing teams comprising of motivated professionals, working out of the hospital setting delivering nursing care in patient's homes and community settings. We have close working networks with our hospital, social and primary care colleagues to support patients to stay in their homes and avoid hospital admission.
Main Duties of the Job
We want to recruit enthusiastic, caring and motivated nurses to provide the best possible care to our patients. If you believe you have these qualities, then we very much look forward to hearing from you! It is essential that you are a car driver and have access to a vehicle for work. Your mileage will be covered by expenses and parking at our community sites is free for clinical staff.

Person Specification
Education & Training
Essential: Registered Nurse; Holds or prepared to work towards gaining an NMC recognised mentor qualification.
Desirable: Registered and up to date mentor; Diploma or degree in healthcare.
Knowledge & Skills
Essential: Post Registration experience of working within Community / or hospital services; Experience of clinical supervision.
Desirable: Relevant skills such as palliative care experience, care of older people.
Skills & Attributes
Essential: Ability to work effectively within an integrated team; Able to plan, deliver and evaluate programmes of care; Ability to prioritise work; Able to communicate effectively.
Desirable: Demonstrates the ability to teach others; Presentation skills.
