Transfer Of Care Hub - Specialist Nurse

[Employer hidden — sign up to reveal] is hiring a Band 6 Nurse for the Transfer of Care Hub at Macclesfield District General Hospital. The role is fixed-term until end of March 2027.

Job summary

An exciting opportunity has arisen for an enthusiastic and forward-thinking Band 6 Nurse to join the Transfer of Care Hub (Integrated Discharge Team) Fixed Term or Secondment until end of March 2027. The successful candidate will promote the Discharge to Assess model by facilitating safe and timely discharges for patients from Macclesfield District General Hospital, supporting the Acute Medical Ward and as necessary for patients in Community Pathway 2 beds Transfer to Pathway 3. The successful candidate will have a passion for putting the patient at the centre and have experience of working collaboratively with multi-professional partners. The job will include supporting the completion of assessments and continuing healthcare checklists within the pathway 2 beds. The post holder will need to be able to travel within Cheshire East to support this. Hours per week: 37.5 hours. We welcome applications from Graduate visa holders; it is important to note that you must have a minimum of 6 months before this expires at the start of your employment for training and operational reasons. Main duties of the job

  • To coordinate and facilitate the safe and effective discharge of patients from Macclesfield District General Hospital supporting proactive reduction in delayed transfer of care.
  • To provide assessments for discharge pathways 1-3.
  • Working proactively within an integrated health and social care team, to promote formal and informal channels of communication both within the team, ward staff, ICB and wider community as appropriate.

Person Specification

Qualifications - Essential

  • Current first level registration in General Adult/Mental Health Nursing.
  • ENB (English National Board) 998 / Clinical Educator Training or equivalent
  • Degree in Physiotherapy or Occupational therapy with experience

Qualifications - Desirable

  • 1st Degree level education (or demonstrates ability to study at this level)

Experience - Essential

  • Multi-agency working
