Care Navigator

[Employer hidden — sign up to reveal] has an opportunity for two Care Navigators to join their friendly, supportive and professional team on a full-time basis, 37.5 hours per week.

About the Role

The Care Navigator is a very important role within the surgery as they are the first person a patient speaks to or sees when they contact the surgery.

  • Greeting and directing all patients and handing out prescriptions, letters, completed forms, and specimen bottles.
  • Making appointments face-to-face, online and via the telephone.
  • Answering the telephone promptly and politely.
  • Managing the needs of the patient, dealing with enquiries and advising of practice procedures.
  • Helping patients access the right services.
  • Ensuring all information is recorded accurately on the computer system.
  • Working as part of the Care Navigator team and supporting all members of the practice staff.

About Us

[Employer hidden — sign up to reveal] is a busy practice in the town centre of Ipswich, Suffolk. We have a practice population of over 20,000 patients. The practice is based on two sites, the main surgery in the centre of the town, and a branch surgery at Stoke Park.

[Employer hidden — sign up to reveal] is part of a PCN (Primary Care Network) with Ivry Street Medical Centre, in Ipswich, and the two practices cover a population of 32,000 patients. The PCN allows the practices to share resources and offer an enhanced service to our patients. The PCN employs care co-ordinators and social prescribers to work with patients, especially our vulnerable patients, to help them to access the best care.
