Community Occupational Therapy Assistant Practitioner

About the Role

[Employer hidden — sign up to reveal] are looking to welcome a Community Occupational Therapy Assistant Practitioner to their team to work on a part-time basis for 30 hours per week. Days can be flexible but must include a Friday.

The post holder will assist in the delivery of high standards of primary care occupational therapy as part of a multidisciplinary team to registered patients within the Primary Care Network footprint, in a range of settings including GP practices, domiciliary settings and care homes.

They will work under the direction and supervision of the PCN occupational therapy lead and reports to and is accountable to the PCN Manager.

Duties will include:

  • Activities of daily living and home assessment
  • Managing a caseload of non-complex patients
  • Supporting patients with mild/moderate frailty - at high risk of needing levels of support in the future with a proactive approach
  • Moving/handling, mobility and falls assessments and interventions
  • Signposting/onward referrals to social prescribing and the voluntary sector to reduce the risk of social isolation.
  • Short term rehabilitation interventions, supervising and progressing exercise programmes
  • Prescribing, supplying, fitting and advising on the use of mobility and activities of daily living equipment
  • Healthy ageing and proactive management of frailty, including generic assessment of nutrition, cognition, medications and falls prevention

About the Candidate

  • GCSE English and Maths grade C or above (or equivalent)
  • NVQ Level 3 desirable
  • Ideally previous experience of working in health and social care setting
  • Experience of working within community health teams, either on placement or as part of your work.
  • Experience of staff supervision and appraisal
