Practice Nurse

The role involves delivering high-quality holistic care, including chronic disease management, childhood vaccinations, cervical cytology, contraception advice, wound care, and infection prevention. The successful candidate will work alongside GPs, pharmacists, and other healthcare professionals. Experience in general practice and autonomous working is essential. The position offers flexibility on hours (30-37.5 per week) and a competitive salary dependent on experience. Sponsorship is available for eligible applicants.

Main Duties of the Job
You will be a qualified and experienced General Practice Nurse with the ability to do smears, childhood vaccinations and contraception advice. In addition to this you will have a good understanding of the main long term conditions patients may have and the experience to undertake reviews for these patients. You will work holistically and be able to advise and support patients to manage their conditions and improve their overall health. You will also support more junior members of the team with their learning and also contribute to the learning of trainee GPNs and other trainees hosted by the Practice.
About Us
We have a list size of 14,000 patients and the nursing team works alongside our GPs, pharmacist team, physiotherapists and other healthcare professionals in the practice to provide for the primary care needs of our patients. We have good administrative support for the nursing team allowing the nurses to focus on direct patient care as much as possible. Our team of HCAs are very experienced and our Lead Nurse is a prescriber who is proactive in supporting the development of her team.
Job Responsibilities
- Support the Infection Prevention Control (IPC) lead and work to the IPC Policy
- Consult with patients and their families to undertake assessments of patient need
- Support in the long-term condition management within the organisation
- Develop, implement and embed an effective care management programme for the frail and housebound patients
- Support public health and screening programmes, including immunisations, vaccinations, and cervical screening
- Provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF
- Support new-to-practice nurses and other healthcare professionals
- Teach and advise patients and their families on how to manage their condition
- Participate in clinical audits and research projects
- Maintain accurate clinical records using SNOMED CT codes
- Lead on public health campaigns (e.g., flu clinics)
- Provide travel advice services and wound care (ulcer/Doppler, etc.)
- Chaperone patients where necessary
- Liaise with external services/agencies to support vulnerable patients
Person Specification
Essential Experience: Broad knowledge of general practice, experience as a Practice or Community Nurse, chronic disease management, working autonomously, infection prevention and control, Practice Nurse skills including wound care, ECG, venepuncture, immunisations, new patient medicals and women's health.
Essential Qualifications: Registered Nurse with the Nursing & Midwifery Council.
Essential Personal Qualities: Effective time management, ability to follow policies, use of PGDs, use own initiative, good interpersonal skills, sensitivity, commitment to professional development.
