Cardiac Rehabilitation Practitioner (Band 6)
[Employer hidden — sign up to reveal] Group is seeking a committed and skilled Cardiac Rehabilitation Practitioner to enhance the quality of life of people living with heart failure. This role plays a key part in delivering high-quality, evidence-based cardiac rehabilitation services across both community and clinical settings. Based in Melksham, Wiltshire, this permanent full-time position offers a salary of £39,959 to £48,117 a year under Agenda for Change terms.
Main Duties
- Deliver timely, high-quality cardiac rehabilitation in line with clinical protocols, service priorities, and professional standards.
- Undertake comprehensive specialist assessments using sound investigative and analytical skills.
- Plan, deliver, and evaluate personalised cardiac rehabilitation interventions in partnership with patients, carers, and MDT colleagues.
- Deliver care flexibly in the most appropriate setting, including clinic and community environments.
- Promote exercise, self-management, and lifestyle change to support long-term health outcomes.
- Monitor outcomes and adapt interventions to optimise patient progress and experience.
- Provide advice, education, and instruction to patients, carers, students, and other health professionals.
- Communicate effectively using a range of methods and systems (including SystmOne and digital platforms).
Leadership and Management
- Work within clinical supervision frameworks, escalating and discussing complex cases with the Clinical Lead.
- Contribute to service planning, development, and quality improvement initiatives.
- Lead on delegated responsibilities and service development projects.
- Communicate effectively with internal teams and external partners across health, social care, and voluntary sectors.
- Maintain competence in the safe use and storage of equipment, in line with service and Trust policies.

Essential Criteria
- Current registration with a relevant healthcare professional body
- Minimum of two years post-registration experience within cardiology and/or cardiothoracic services
- Experience in delivering education or teaching to patients, carers, students, or colleagues
- Experience of evaluating and contributing to the improvement of clinical care
- Sound knowledge of national policies and guidance influencing service delivery (e.g. NICE)
- Strong interpersonal and communication skills
- Ability to work effectively both independently and as part of a multidisciplinary team
- Awareness of research principles and evidence-based practice
- Full, valid UK driving licence
- Access to a roadworthy, insured, and MOT-compliant vehicle
Desirable Criteria
- Educated to, or currently working towards, Masters level qualification
- Experience of supporting or delivering health promotion activities
