Job summary
[Employer hidden — sign up to reveal] are seeking a Community Staff Nurse to join our friendly and supportive team in the Blandford team. The post is full time. You would work alongside our experienced Community Nurse Teams helping to provide a high-quality standard of care for patients in their own homes. We can be flexible with shift patterns as long as the service needs are met between hours of 08:00-20:00 with some attractive weekend enhanced pay. We offer a structured induction programme, flexible working patterns and pastoral support. There are excellent learning and development opportunities, both in-house and through our links with local universities, plus the option of a preceptorship programme for newly qualified nurses to boost your skill development.

Main duties of the job
- Provide a high-quality person-centred approach to care delivery considering safety, privacy, dignity while promoting the Trust's values.
- Undertake assessment of patients care needs and develop programmes of care as part of the wider multi-disciplinary care team.
- Manage acute and chronic nursing needs, develop strong therapeutic relationships with patients, work closely with community partners.
- Identify care needs, monitor conditions, record observations, help patients develop wellbeing care plans.
- Support people to maintain wellbeing and independence, avoid unnecessary hospital admissions, deliver exceptional end of life care including symptom management.

Person Specification
Knowledge, Skills and Training
Essential
- Registered practitioner to degree/diploma level
- Membership of the relevant Professional Body
- Evidence of recent professional development in an up to date portfolio
- Knowledge and understanding of Trust Strategy and policies relevant to role
Job Specific Experience
Essential
- Supervised experience gained during clinical placements
- Ability to mentor students / supervise other junior staff
- Demonstrable knowledge of assessment and therapeutic interventions in area of practice
- Experience of contributing to the management of change
Desirable
- Recent previous experience within a comparable role
Managerial Experience
Essential
- Experience of demonstrating duties to junior staff
Desirable
- Experience of providing clinical supervision and mentoring to junior staff
Information Technology
Essential
- Able to evaluate data and produce reports using Microsoft Excel and Word
- Experience of using electronic patient / service user record systems
