Health and Social Care Coordinator
[Employer hidden — sign up to reveal] is seeking a Health and Social Care Coordinator to join Adult Community Services in West Kent. This role involves coordinating care for housebound patients, managing a caseload, and working with multiple partners. The position is part-time (22.5 hours/week), permanent, based at Heathside Centre, Coxheath, ME17 4AH. Salary: £28,392 to £31,157 a year pro rata per annum (Band 4 Agenda for Change). Closing date: 11 May 2026.
Main Duties
- Act as key point of contact for patients, carers, and professionals
- Support complex health and social care needs
- Work across Acute Trusts, Primary Care, Social Services, and voluntary organisations
- Help reduce unnecessary hospital admissions via referrals and advice
- Manage own caseload and plan outcomes
- Requires excellent communication, organisation, and analytical skills
Person Specification
- Level 4 qualification or equivalent experience
- Functional Skills Level 2 (GCSE English and Maths A-C/4-9)
- Recent team working experience
- Full UK driving licence or EU licence (not exchanged from non-EU)
- Sound communication skills
- Customer-focused approach
- Ability to use IT systems
