Job summary

We are seeking a dedicated Practice Nurse to join our healthcare team, initially on a one year fixed term contract. The successful candidate will provide high-quality patient care within a general practice setting, supporting doctors and other healthcare professionals. This role offers an excellent opportunity to utilise your nursing skills in a dynamic environment, contributing to the wellbeing of our patients. The Practice Nurse will be responsible for delivering comprehensive nursing services, including health assessments, immunisations (including baby immunisations, smears and chronic disease management). This role will work across both sites (Rye Road and Talbot Road) working Monday and Friday at Rye Road and Tues and Wednesday at Talbot Road. 32 hours per week, 8.30am-5.30pm.

Main duties of the job

Responsibilities

  • Conduct patient assessments and health checks with professionalism and compassion
  • Administer vaccinations and immunisations in accordance with national guidelines
  • Assist with minor procedures and provide pre- and post-procedure care
  • Support patients with chronic disease management programmes, including monitoring and education
  • Maintain accurate and detailed patient records in compliance with data protection regulations
  • Provide health promotion advice and education tailored to individual patient needs
  • Collaborate closely with GPs, practice staff, and other healthcare professionals to ensure seamless patient care
  • Ensure infection control protocols are strictly followed within the practice environment

Requirements

  • Qualified Practice Nurse registered with the Nursing and Midwifery Council (NMC)
  • Proven experience in patient care within a primary care or similar healthcare setting
  • Already qualified in giving baby immunisations
  • Qualified (with PIN) to carry out smears
  • Strong knowledge of anatomy and physiology relevant to nursing practice
  • Excellent communication skills, demonstrating empathy and professionalism when engaging with patients
  • Ability to work effectively both independently and as part of a multidisciplinary team
  • Organised, detail-oriented, with the ability to manage multiple priorities efficiently
  • Commitment to ongoing professional development and adherence to clinical guidelines

Job summary: To provide and maintain a high standard of nursing care for patients, as well as providing nursing assistance to the doctors and other members of the primary healthcare team. The duties will include all tasks normally undertaken by an experienced RGN and any additional roles agreed between the nurse and the doctors as appropriate, having regard to current training.
Job responsibilities:
Diagnosing and treating patients presenting with minor illness

  • Triage and treat patients wishing to see a health care professional, making any necessary referrals to other members of the primary health care team
  • Ensure clinical practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.
  • Advise patients on general health care and minor ailments, with referral to other members of the primary and secondary health care team as necessary

Pathological specimens and investigatory procedures

  • Undertake the collection of pathological specimens including intravenous blood samples, swabs etc. Perform investigatory procedures requested by the GPs

Administration and professional responsibilities

  • Participate in the administrative and professional responsibilities of the practice team
  • Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes
  • Ensure the clinical computer system is kept up to date, with accurate details recorded and amended
  • Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator
  • Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice
  • Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit
  • Attend and participate in practice meetings as required
  • Restocking and maintenance of clinical areas and consulting rooms
  • Recording of fridge temperatures
  • Maintenance of the log book for the resuscitation equipment and anaphylaxis kit
  • Updating of policies, procedures and protocols
  • To be the infection control lead in the practice and ensure all staff are aware of current procedures

Supplies and equipment Maintenance of equipment and stock relating to patient care

  • Maintenance of supplies and cold chain relating to vaccines

Training and personal development

  • Training requirements will be monitored by yearly appraisal and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments.
  • Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development, ensuring PREP requirements are met.
  • If it is necessary to expand the role to include additional responsibilities, full training will be given.
  • Develop and maintain a Personal Learning Plan

Liaison

  • As well as the nursing team, there is a need to work closely with reception, office and dispensary staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person and ensuring everyone is aware of the different roles within the nursing team.
  • There is also the need to establish and maintain good liaison with other surgeries and agencies, including secondary care
