Experienced Practice Nurse
[Employer hidden — sign up to reveal] is hiring an Experienced Practice Nurse at Ranworth Surgery in Clacton on Sea. This is a part-time, permanent role for 13.5 hours per week, working Monday 08:30-18:30 and Tuesday 08:30-13:00. The post holder will manage long-term conditions, support frail and housebound patients, and work as a senior member of the multidisciplinary team.
Main Duties
- Support the production of the IPC annual statement in conjunction with the nurse manager.
- Develop, implement and embed an effective care management programme for the frail and housebound patients.
- Assess the needs of patients ensuring evidence-based practice.
- Develop, implement and embed health promotion and wellbeing programmes.
- Implement and evaluate individual treatment plans for chronic disease patients.
- Identify, manage and support patients at risk of developing long-term conditions.
- Diagnose and manage acute and chronic conditions.
- Review patient medications to enhance compliance.
- Support the clinical team.

Person Specification
Experience
Essential:
- Experience in a primary care environment.
- Chronic disease management.
- Infection prevention and control experience.
- Continuing professional development (CPD).
- Flexibility to work outside office hours.
Desirable:
- Experience as a community nurse.
Qualifications
Essential:
- Registered nurse with current NMC registration and active PIN.
Desirable:
- Mentor or teaching qualification.
- Nurse prescriber.
- Post graduate diploma or degree in chronic disease management.
- Family planning qualification.
