FULL DESCRIPTION

Experienced Practice Nurse

[Employer hidden — sign up to reveal] is hiring an Experienced Practice Nurse at Ranworth Surgery in Clacton on Sea. This is a part-time, permanent role for 13.5 hours per week, working Monday 08:30-18:30 and Tuesday 08:30-13:00. The post holder will manage long-term conditions, support frail and housebound patients, and work as a senior member of the multidisciplinary team.

Main Duties

  • Support the production of the IPC annual statement in conjunction with the nurse manager.
  • Develop, implement and embed an effective care management programme for the frail and housebound patients.
  • Assess the needs of patients ensuring evidence-based practice.
  • Develop, implement and embed health promotion and wellbeing programmes.
  • Implement and evaluate individual treatment plans for chronic disease patients.
  • Identify, manage and support patients at risk of developing long-term conditions.
  • Diagnose and manage acute and chronic conditions.
  • Review patient medications to enhance compliance.
  • Support the clinical team.

Person Specification

Experience

Essential:

  • Experience in a primary care environment.
  • Chronic disease management.
  • Infection prevention and control experience.
  • Continuing professional development (CPD).
  • Flexibility to work outside office hours.

Desirable:

  • Experience as a community nurse.

Qualifications

Essential:

  • Registered nurse with current NMC registration and active PIN.

Desirable:

  • Mentor or teaching qualification.
  • Nurse prescriber.
  • Post graduate diploma or degree in chronic disease management.
  • Family planning qualification.
