Main Duties of the Job

You will work autonomously, using advanced decision-making skills to deliver safe, effective, and patient-centered care in urgent and primary care settings. Responsibilities include providing advanced clinical care through telephone triage, remote consultations, and face-to-face assessments, diagnosing and managing complex cases, prescribing medications, and coordinating care across services. You will also lead on clinical quality and safety, mentor colleagues, and safeguard vulnerable individuals. Job Responsibilities
- Provide advanced clinical care through telephone triage, remote consultations, and face-to-face assessments for patients with undifferentiated and urgent health needs, including paediatrics, elderly care, and mental health.
- Diagnose and manage complex cases using advanced assessment, examination, and clinical reasoning skills.
- Prescribe and manage medications as an independent prescriber, ensuring safe and effective treatment plans.
- Order and interpret investigations (laboratory and radiology) and make evidence-based decisions on care management.
- Coordinate care across services, acting as a referral agent and care coordinator to ensure seamless patient journeys.
- Lead on clinical quality and safety, promoting best practice, supporting audits, and contributing to service improvement initiatives.
- Mentor and support colleagues, providing clinical supervision, coaching, and contributing to the development of junior staff and students.
- Safeguard vulnerable individuals, applying policies and legislation in child protection, mental health, and safeguarding adults.
- Work flexibly, including evenings, weekends, and bank holidays, and travel to different [Employer hidden — sign up to reveal] sites as required.
What We Expect
- Professional registration (NMC, HCPC, GPhC) with post-qualification experience.
- Advanced assessment and minor illness qualifications (Level 6 or 7).
- Independent prescribing qualification (Essential).
- Proven experience in autonomous practice and clinical leadership.
- Strong communication skills, able to manage complex and sensitive situations.
- Ability to prioritise, problem-solve, and make decisions under pressure.
- Commitment to continuous professional development and reflective practice.
