Job Summary

There is an exciting opportunity to join the community Nursing Teams based in Mildenhall (and at our satellite base at Brandon) as a Band 6 Junior Sister. The nursing teams form part of the Integrated Neighbourhood Team (INT) and would see the successful applicant joining one of our dynamic and friendly team of neighbourhood based community nurses, therapists, community matrons, EIT and Virtual ward, and colleagues within social care. You will be providing (urgent/virtual and preplanned) nursing services to patients in the community and have the chance to work in an autonomous manner with the support of the wider multidisciplinary team including the Community Matrons, tissue viability team and diabetic nurse specialists. Within your role you will be able to provide clinical and supervisory leadership to staff and students within the team. Supporting the management of the nursing staff by taking responsibility, in partnership with the District Nurse, for the day-to-day co-ordinating, planning and delivery of the nursing services within the INT. You will be supporting the management of patients with multiple and complex needs, using evidence-based practice to assess, plan, implement and evaluate interventions to improve patient health. Our aim is to ensure the patients are provided quality cared within their neighbourhood/locality as a well-integrated team.

Main Duties of the Job

Your role will be: Experience in leadership as well as clinical experience in urgent care, virtual ward/hospital at home care, palliative care, and preplanned care (such as wound management, diabetic care, catheter ongoing management). INT quality improvement project work and conducting monthly clinical audit will also be a key part of this role.

  • To provide clinical and supervisory leadership to junior staff and students within the team.
  • To be responsible for the day-to-day planning and delivery of the nursing services within the Community Healthcare Team.
  • To manage a caseload of patients with multiple and complex needs, using evidence-based practice to assess, plan, implement and evaluate interventions, with the patient at the centre.
  • To provide specialist Nursing input into planning, development and evaluation of clinical services within the team.
  • To support change and drive quality improvement through auditing.

Person Specification

Education & Qualification:

  • Essential: Current NMC registration or working towards. Evidence of continuing professional development recent and relevant to the post. Leadership training. Level 3 degree Clinical module on long term conditions such as Leg Ulcer Management, Diabetes care or End of Life Care. Care Lead / Case Management /Health coaching training.
  • Desirable: Demonstrate knowledge, understanding and use of HR management policies including Capability, Sickness management, and Disciplinary.

Experience & Knowledge:

  • Essential: Evidence of working within the community nursing setting. Experience of supervision of staff, including PDR. Evidence of high level of ability to reflect on and critically appraise own practice and performance. Experience of co-ordinating and delegating work and reviewing others performance. Experience and knowledge of autonomous and independent decision making in home environments, which are often complex and unpredictable. Enhanced awareness of mental health conditions and issues. Comprehensive holistic assessment skills in the patient's home environment.
  • Desirable: Evidence of formal clinical supervision. Ability to give constructive feedback and praise.

Skills & Abilities:

  • Essential: High level of nursing skills and knowledge for managing and supporting the care of people in their own home environment. High level of ability to work under pressure and set priorities to achieve service objectives. High level of clinical reasoning skills and credibility as a nursing leader. Knowledge of additional specialist areas through in-house training and short courses. Effective communication skills with people on complex matters, issues, ideas, in complex situations. Ability to work alone and give professional supervision and guidance to junior staff. Ability to act on own initiative and manage any risks. Good observational and reporting skills. Able to give examples of working flexibly to accommodate patient/service needs. The ability to drive to rural locations and hold a current valid driving licence. Ability to manoeuvre patients using handling aids.
  • Desirable: Cannulation / IV Therapy administration.
