As the Council continues to transform Adult Social Care, the Hospital Discharge Manager within the Hospital Discharge Team plays a central role in ensuring that every resident with care and support needs can live a Better Life in the place they call home.
The Importance of the Role
The Hospital Discharge Manager plays a vital role in supporting people to leave hospital safely, promptly and with the right support in place, promoting reablement-led pathways wherever possible. By championing a Home First and Discharge to Assess approach, the role ensures individuals are supported to maximise independence and avoid unnecessary long-term care. Through effective leadership and partnership working across health, social care and the voluntary sector, the postholder helps reduce delayed discharges, improve system flow and embed strengths-based, reablement-focused practice across the discharge pathway. This role is central to delivering integrated, person-centred care and supporting prevention agendas across the wider health and care system.

- Professional qualification and registration as a Social Worker
- Strong understanding of legislation, safeguarding, and best practice
- The ability to supervise, motivate, and develop staff
- A balanced approach to risk, rights, and independence
- Creativity in meeting need and problem-solving
- Strong partnership working across health, community, and voluntary sectors
- A commitment to person-centred, strengths-based practice
- Confidence and capability to deputise for the Team Manager when required
