Job summary

You will play a key role in delivering our vision of accessible, highquality 24/7 general practice, supporting patients to receive the right care, in the right place, at the right time. Working as part of a multidisciplinary team you will be trusted to make complex clinical decisions, manage undifferentiated presentations, and influence how services are designed and delivered across the system. Main duties of the job
The Advanced Clinical Practitioner (ACP) is a senior autonomous clinician who delivers high-quality, evidence-based care across [Employer hidden — sign up to reveal]'s integrated primary care and urgent care services, supporting the organisations vision of accessible, 24/7 general practice. The post holder will assess, diagnose, treat and manage patients with undifferentiated, acute and complex conditions, while contributing to service development, clinical leadership, education and quality improvement across the system.

Job responsibilities
- Practise as an independent and autonomous clinician
- Undertake comprehensive clinical assessment, diagnosis and management
- Prescribe and manage medications safely
- Work collaboratively within a multidisciplinary team
- Provide visible clinical leadership
- Maintain advanced clinical competence
Person Specification
- Experience as an Independent Prescriber
- Advanced Clinical Practice (ACP) qualification MSc level
- Registered Non-Medical Independent Prescriber
- Safeguarding adult and children Level 3
- Minimum 5 years experience post qualification
