Advanced Care Practitioner
[Employer hidden — sign up to reveal] Limited is seeking an Advanced Care Practitioner to work as a senior autonomous clinician in an integrated primary and urgent care model. The role involves comprehensive clinical assessment, diagnosis, treatment, prescribing, leadership, education, and service development. Requires MSc in Advanced Clinical Practice, NMC or HCPC registration, independent prescriber qualification, and minimum 5 years post-qualification experience.
Location: Hereford, UK | Contract: Permanent | Salary: Competitive rates dependent on experience

Main Duties
- Practise as an independent and autonomous clinician managing undifferentiated, acute and chronic conditions.
- Undertake comprehensive clinical assessment, diagnosis and management with advanced clinical reasoning.
- Initiate, interpret and act on investigations and referrals.
- Prescribe and manage medications safely and effectively.
- Contribute to flow, continuity and demand management across integrated services.
- Work collaboratively with GPs, nurses, paramedics, pharmacists, and operational teams.
- Provide visible clinical leadership and support service development.
- Maintain advanced clinical competence and support education of colleagues.

Person Specification
- RN current NMC registration or Paramedic HCPC registration.
- Advanced Clinical Practice (ACP) qualification at MSc level or equivalent.
- Registered Non-Medical Independent Prescriber.
- Safeguarding adult and children Level 3.
- Minimum 5 years experience post qualification.
- Experience in managing undifferentiated undiagnosed conditions.
- Excellent communication skills and IT literacy.
- Driver with clean licence.
