Job Summary
The Urgent Community Response (UCR) Service provides urgent support, seven-days a week, to prevent unnecessary hospital admissions. The role involves providing a two-hour crisis response delivered by a multi-skilled team at home or where the patient resides. The service also provides general virtual ward support.
Main Duties of the Job
The postholder will work within a multidisciplinary team comprising GPs, Nurses, Paramedics, Pharmacists, Occupational Therapists, Physiotherapists, and Health & Therapy Assistants. Key responsibilities include:
- Providing clinical interventions including intravenous therapy as part of admission avoidance pathways (2-hour response and general virtual wards)
- Triaging, assessing, and delivering care to patients over several days to ensure safe care at home
- Providing clinical expertise and problem-solving support for the team
- Coordinating the multi-disciplinary team and allocating visits
- Developing the service through reflective practice, clinical supervision, and audit
- Identifying suitable people for planned early discharge or prevention of admission

Person Specification
Qualifications
- Professional qualification (degree or equivalent) in Physiotherapy or Occupational Therapy
- Current registration with the Health and Care Professions Council
- Evidence of relevant postgraduate courses/CPD and reflective practice
Experience
- Substantial relevant post-qualification experience
- Experience of interdisciplinary working
- Experience of supervising junior staff, assistants, and students
- Experience of independently managing a clinical caseload
Skills and Knowledge
- Competency in a range of clinical skills
- Able to plan and prioritise own workload
- Able to delegate work to others
- Able to demonstrate leadership in practice
Other Requirements
- Car driver (Valid UK Driving License) and access to a vehicle
Salary and Contract
Salary: £39,959 to £48,117 a year (Band 6). Contract: Permanent. Working pattern: Full-time, Part-time. Pay scheme: Agenda for Change.
