SEND Keyworker
[Employer hidden — view at passion-project.co.uk] is seeking SEND Keyworkers to support Family Practitioner Leads in coordinating EHC assessments, producing EHC plans, and conducting annual reviews for children with special educational needs. Hybrid working available, with full-time and part-time positions in Newcastle, Staffordshire Moorlands, and wider council areas.

Main Responsibility
- Supporting the Family Practitioner Leads to carry out the Local Authority’s SEND duties in relation to co-ordinating EHC Assessments
- Producing EHC Plans and carrying out annual reviews
- Working closely with our children and young people with special educational needs and disability and their families as well as a range of qualified professionals in education, health and social care.
- Developing collaborative and respectful relationships, promoting choice and independence and improve outcomes
- Ensuring that education, health, and care plans comply with legal, regulatory, ethical and social requirements in addition to the policies, standards and procedures of the authority.
The Ideal Candidate
- Ability to manage your own caseload independently and as part of the team.
- Excellent written and verbal communication skills
- Knowledge of the SEND legislation
- Ability to reflect on practice and improve as a result.
