Job Description

We're now looking for a Band 7 Occupational Therapist to join us as a key contributor to our discharge and transition planning function. What You'll Do

  • Provide advanced OT input into complex discharge planning
  • Support the development of structured transitional programmes
  • Work closely with MDTs and external partners to unblock barriers and improve flow
  • Contribute to service improvement projects focused on progression and discharge
  • Supervise and develop the OT workforce in discharge‑focused practice
  • Lead on outcome measurement and evidence‑based pathway development

Who You Are

A confident, skilled OT with strong experience in mental health or secure settings, excellent clinical reasoning, and a passion for improving patient pathways and transitional care.
