The role involves managing a clinical caseload, delivering training, and ensuring high-quality care for clients in their own homes.

About the Role

The Complex Care Nurse is an integral part of the excellent care delivery [Employer hidden — sign up to reveal] provide to our adult and child social care clients. The role will manage a client caseload whilst collaborating with stakeholders and other healthcare services to ensure specialised care is delivered to the highest possible standard.

Key responsibilities include:

  • Accountable for clinical components of care delivery
  • Support assessment, mobilisation and implementation of complex care clients
  • Regularly risk assess individual client needs
  • Provide evidence-based clinical expertise and advice
  • Attend MDT meetings and liaise with commissioners, case managers and clients
  • Act as a role model and leader for all staff
  • Deliver clinical training and competency assessments to non-registered carers
  • Maintain high standards and manage quality improvement, clinical risks, safety concerns and complaints

Essential Requirements

  • Registered Nurse with NMC and active PIN
  • Minimum 2 years clinical nursing experience (community or hospital)
  • Management of clinical risk and person-centred care planning
  • Experience safeguarding adults and children
  • Effective communication skills (verbal and written)
  • Ability to work on own initiative and as part of a team
  • People management skills
  • Excellent IT and presentation skills
  • Valid UK driving licence (desirable)
