Job summary
PMLs Collaborative Care Team (CCT) supports seventeen General Practices located within West Northamptonshire, providing proactive assessment and support to people with longterm and complex conditions and their Carers. We undertake holistic assessment within the home environment, identifying needs and priorities with patients and their carers to improve wellbeing, maintain independence with an overarching aim to prevent unplanned admissions to hospital.
The Team currently comprises two Registered Nurses (RNs), an Occupational Therapist (OT), two Assistant Practitioners (AP), a Social Prescribing Link Worker and a Patient and Service Coordinator. We also provide practice placements for Pre-Registration AHP students from the University of Northampton. The CCT operates Monday to Friday, 08:30 to 16:30 excluding Bank Holidays

Main duties of the job
The therapist is responsible for delivering high-quality, evidence-based therapeutic interventions to support patient recovery, wellbeing and independence. The role contributes to the delivery of core therapeutic services, working both independently and as part of the multidisciplinary team to ensure safe, effective and person-centred care in line with organisational and [Employer hidden — sign up to reveal] standards.
The postholder applies clinical knowledge and experience to assess needs, determine appropriate interventions and manage day-to-day clinical issues. To escalate only complex or high-risk situations.

Job responsibilities
- Clinical Care Delivery - Deliver safe, effective and evidence-based therapeutic interventions in line with individual care plans, professional standards and organisational policies. Use clinical judgement to assess patient needs and determine appropriate treatment approaches within established frameworks. Manage a defined caseload, prioritising interventions to meet service and patient needs.
- Patient Safety and Quality - Apply and interpret clinical governance requirements. To include safeguarding, risk management and infection prevention and control. Identify and respond to risks, escalating complex or high-risk situations appropriately. Contribute to maintaining high standards of care and continuous quality improvement.
- Documentation and Record Keeping - Maintain accurate, timely and confidential patient records in accordance with [Employer hidden — sign up to reveal], legal and professional requirements. Ensure documentation supports clinical decision-making and continuity of care.
- Multidisciplinary Team Working - Work collaboratively with the multidisciplinary team, contributing to assessments, reviews and shared decision-making. Coordinate aspects of patient care with colleagues and services to support effective care pathways. Build effective working relationships with patients, carers and professionals to support positive outcomes.
- Communication and Patient Engagement - Communicate effectively with patients and carers, providing advice, education and support to enable engagement in therapy and promote independence. Use interpersonal and influencing skills to support behaviour change and adherence to treatment plans.
- Professional Practice and Development - Maintain professional competence through supervision, reflective practice and continuing professional development. Keep up to date with relevant clinical guidelines, policies and best practice. To apply these to day-to-day work. - Holistic Clinical Assessment and Care Delivery - Deliver evidence-based, frailty-focused physiotherapy assessment and intervention in patients homes to improve wellbeing, maintain independence, and reduce unplanned hospital admissions.
- Supervisory Leadership and Professional Oversight - Provide structured clinical and professional supervision to ensure safe, high-quality practice, support staff competence and wellbeing, uphold governance and regulatory standards, and contribute to continuous service improvement in line with [Employer hidden — sign up to reveal] requirements.
- Clinical Leadership and Operations Support - Provide physiotherapy leadership, professional guidance, and delegated operational support to ensure safe, effective, and high-quality team performance.
- Caseload Management and Integrated Working - Manage and prioritise a defined caseload while collaborating effectively with the multidisciplinary team, GPs, patients, carers, and partner agencies.
- Quality, Governance and Compliance - Contribute to audit, QOF, CQC compliance, policy development, and appraisal processes to maintain high standards of clinical governance.
- Workforce and Service Development - Support the development of staff, students, and physiotherapy services through mentoring, training, and active contribution to integrated care pathway development.
Person Specification
Skills and knowledge
Essential
- Knowledge of evidence-based therapeutic practice and relevant clinical guidelines
- Ability to undertake holistic assessment, apply clinical reasoning, and deliver evidence-based physiotherapy interventions tailored to individual and community needs
- Sound knowledge of consent, capacity, and legal frameworks, ensuring safe practice alongside accurate documentation and compliance with professional standards
- Ability to communicate complex clinical information effectively to patients, carers, and professionals to support coordinated, patient-centred care
- Ability to prioritise and manage own and delegated caseloads, maintain accurate records, and contribute to service monitoring and resource management
- Problem-solving skills, with the ability to manage routine clinical issues and escalate where appropriate
Qualifications
Essential
- Recognised Physiotherapy qualification to BSc or accelerated MSc level
- Active registration with the HCPC
Desirable
- Additional accredited training in a specialist therapy area
Experience
Essential
- Experience delivering therapeutic interventions within a healthcare, community or NHS provider setting
- Experience managing or contributing to a defined caseload and prioritising patient interventions
- Experience working with patients as part of a multidisciplinary team.
