Senior Intensive Outreach Clinician

Job Summary

The post holder will offer early assessment, diagnosis and treatment for service users meeting criteria for Assertive Community Treatment. The role includes person-centred care, trauma-informed approaches, collaborative teamwork, and proactive support for carers. The service aims to maximise symptomatic and social recovery.

Main Duties

  • Provide pathway leadership and effective coordination between staff and disciplines.
  • Ensure timely, high-quality holistic wrap-around support for service users.
  • Connect the pathway with community partners and services.
  • Work with single point of access and other interfaces for smooth transitions.
  • Oversee governance, quality processes, and data capture.
  • Provide supervision to pathway staff as agreed with the Service Manager.
  • Ensure effective internal and external communication processes.

Person Specification

Qualifications

  • Professional Qualification (RMN, OT, SW) with current professional registration.
  • Evidence of mental health training and continued professional development.
  • Educated to degree level or equivalent.
