Main duties of the job
- Carrying out routine physical health monitoring: observations of blood pressure, pulse, temperature, height, weight and BMI.
- Supporting phlebotomy pathways and blood sampling.
- Conducting ECGs and contributing to wider clinical monitoring.
- Promoting healthy lifestyle interventions.
- Supporting the Physical Health Hub & Spoke model.
- Working collaboratively within a multidisciplinary team.
- Accurately recording and maintaining clinical information.
- Supporting service users with personalised, recovery focused care.

Job responsibilities
The purpose of this role is to provide high quality, person centred physical health support to individuals under Core Mental Health Services (CMHTs). This includes routine physical health monitoring, supporting phlebotomy, ECGs, promoting healthy lifestyle interventions, and delivering care in line with the Physical Health Monitoring Model. The post holder will work under daily supervision of mental health practitioners as part of the multidisciplinary team.
Communication and Working Relationship Skills
- Work closely with Registered Nurses, Pharmacists, GPs, VCSE partners and MDT colleagues.
- Participate in MDT meetings.
- Communicate sensitively with people who may experience anxiety, neurodiversity, psychosis or health related distress.
- Record all results in Physical Health Launchpad, SystmOne or EMIS.
- Offer emotional support and signposting to services.
Analytical and Judgemental Skills
- Work within own limits and competencies.
- Participate in risk assessment and management.
Planning and Organisational Skills
- Deliver brief interventions and health promotion conversations (smoking cessation, weight management, diet, etc.).
- Maintain accurate records and liaise with agencies.
- Monitor mental health and report changes.
Responsibility for Patient/Client Care
- Carry out routine physical health observations including NEWS2 scoring, waist circumference, metabolic monitoring.
- Support venepuncture pathways and ECGs.
- Support clozapine and depot monitoring clinics.
- Escalate abnormal findings to registered practitioners.
- Staff physical health monitoring clinics and provide outreach visits.

- Contribute to improving the PHM model.
- Participate in audits and data quality improvement.

- Attend mandatory training and maintain competencies in observations, venepuncture, ECG, infection control.

Person Specification
Physical Skills
- Ability to carry out physical health observations accurately
- Good communication skills
- Ability to work independently and within MDT
- Understanding of confidentiality, safeguarding, infection control
- Basic IT skills including electronic record systems
- Mobility throughout the trust area
Knowledge and Skills
- Knowledge of mental health and basic understanding of physical health presentations
- Awareness of safeguarding principles
- IT skills
- Ability to make and sustain professional therapeutic relationships
- Good communication and listening skills
- Problem solving skills
Experience
- Experience working in health or social care
- Experience supporting individuals with mental health needs
- Experience providing direct care or health support activities
- Desirable: experience in community or clinic based physical health services, recording clinical data in electronic systems
Qualifications
- GCSE English and Maths or equivalent
- Care Certificate (or willingness to complete within 6 months)
- Willingness to undertake venepuncture, ECG, and physical health training
- Desirable: NVQ Level 3 in Health & Social Care, previous training in physical health monitoring, phlebotomy, or ECG
