Registered Nurse

[Employer hidden — sign up to reveal] is hiring a Registered Nurse for the ICAR unit at Houghton Primary Care Centre. This is a part-time permanent position (36 hours per week).

Job summary

We are currently in the position to recruit a Registered Nurse to ICAR (Intermediate Care and Rehabilitation) unit. This is an excellent opportunity for an individual with an interest and desire to contribute to the delivery of high-quality nursing care to patients with complex care and rehabilitation needs. ICAR is a 24 bedded nurse led rehabilitation unit, offering medical, nursing and therapy assessment and providing patient centred care to optimise independence, recovery and patient wellbeing. You will be part of the multi-disciplinary team, consisting of medical staff, Pharmacists, Nurse Practitioners, Physiotherapists, Occupational Therapists, Nurses and Health Care Assistants providing high quality holistic care to patients and support to their families. The team is welcoming, friendly and supportive and the successful applicant will be fully supported. You will be responsible for the delivery of individualised patient care, through assessment, care planning and evaluation of care and be able to make clinical decisions with a sound knowledge base associated with this patient group. You will be expected to maintain agreed standards of care, directly protecting and promoting patients dignity, choices, self-esteem and well-being at all times with compassion and commitment.

Main duties of the job

  • 1st Level Nurse.
  • The desire to make a difference in this clinical environment with an understanding of the complexity of frail, elderly patients and those requiring rehabilitation.
  • Experience in a hospital/care environment.
  • Demonstrable communication skills.
  • Comprehensive patient assessment and care planning.
  • Direct hands-on nursing care providing or assisting with patient activities of daily living.
  • Follow colleagues mobility plans to promote independence and self-care.
  • Monitor patient progress and have a maintain a high standard of documentation.
  • Good organisational skills.
  • The ability to work as part of a team.
  • Able to move and handle safely without causing harm to yourself or others.

  • To be responsible and accountable for the delivery of safe, effective and compassionate nursing care for patients designated to their care.
  • To always work to the NMC Code.
  • To supervise non-registered staff as stipulated by the NMC under the requirements of delegation.
  • Provide effective leadership and assist with the management and organisation of the ward/department in the absence of the Ward Manager or Junior Sister / Charge Nurse.
  • To support patients who are deemed community ready, in preventing deconditioning.
  • To comply with the trusts vision, values and behavioural compact.

Person Specification

Experience

Essential: Experience of working in an acute hospital or community healthcare setting. Desirable: Recent experience of working as a registered nurse.

Qualifications

Essential: Current NMC Registration (Level 1 adult nurse) or evidence of qualification if newly qualified. Diploma in health related study or equivalent appropriate qualification. Desirable: Degree in nursing studies or healthcare related subject (UK equivalent). Mentorship certificate/ Practice Assessor training or demonstrable equivalent experience.

Physical Skills

Essential: Ability to assist patients to move as per Trust Moving and Handling Policy.

Skills and Knowledge

Essential: Ability to provide and receive complex, sensitive or contentious information. Able to work without supervision. Competent in the use of IT such as Microsoft Office. Evidence of CPD. Knowledge of national health and social care policy. Knowledge of evidence based guidelines e.g. NICE guidelines, NPSA alerts, current nursing legislation. Desirable: Venepuncture and cannulation competency.
