Job Summary

Are you looking for the opportunity to work within an award winning specialist Community Falls & Rehabilitation Team? We are looking for a skilled and enthusiastic Therapy Assistant to join our multidisciplinary team providing rehabilitation for adults with a range of conditions impacting their functional ability.

Our friendly and supportive team is comprised of physiotherapists, occupational therapists, specialist nurses & therapy assistant practitioners, who are all passionate about patient centred care and supporting individuals to regain, retain & improvement their functional independence.

Having won a NICE Shared Learning Award & finishing runner up in the CAHPO awards 2019 for the developments to our Falls service, the team are listed as an example of good practice: Dudley Falls Prevention Service | NICE. This demonstrates our commitment to evidence-based, multifactorial assessment & interventions for our diverse and often complex caseload.

As a service we have built upon this success, embedding best practice & seeing the rate of falls related admissions remain below the national & local average.

The service works closely alongside the local Rapid Response Team- Own Bed Instead, and Intermediate Care Team colleagues, with opportunities for collaborative working to optimise patients' rehabilitation journeys.

The role will work delivering tailored, specialist treatment plans both in patients' homes and from our specialist Rehab Centre at Corbett Hospital.

Main Duties of the Job

  • To work as part of the Community Rehab and Falls Team providing and progressing effective and efficient rehabilitation programs set by the therapists to patients with a variety of physical conditions, as part of an MDT.
  • Duties will include basic assessment, ongoing reassessments, delivery and progression of therapy programs to patients in their places of residence and in clinic setting as required.
  • To work with therapists as a second pair of hands as required and support with admin tasks as necessary and delegated.
  • To Assist with the direct care of patients as delegated to and supervised by registered Allied Health Professional.
  • To provide support to the team working collaboratively with members of the multi-disciplinary team and contributing positively to the delivery of high quality compassionate care.
  • To uphold the fundamental standards of safety and quality always putting the patients first and protecting them from harm.
  • Demonstrates the values of the NHS and commitment to personal and Professional Development.

Person Specification

Qualifications & Education (Essential)

  • Foundation degree Health and Social Care, or equivalent level 5 qualification OR NVQ Level 3 Health + social Care with equivalent clinical experience
  • 5 GCSEs or equivalent, including Functional Skills (level 2) Maths & English

Knowledge and Training (Essential)

  • Able to demonstrate substantial experience in health and care settings, including in the relevant field of work.
  • IT Training / knowledge
  • Good understanding of rehabilitation
  • Commitment to undertake appropriate training

Experience (Essential)

  • Core knowledge of clinical, technical and/ or scientific roles and responsibilities, including clinical risk assessment
  • Relevant clinical experience eg community, falls, general rehab, elderly

Communication & Relationship Skills (Essential)

  • The ability to communicate clearly, respectfully, and effectively using a range of methods - written, verbal and non-verbal
  • Maintaining confidentiality and protection of data and overcoming physical, sensory, cognitive and language barriers to understanding
  • Demonstrates the ability to provide and receive complex and sensitive information, including relating to service users. Judges the appropriate method of communication, ensuring information is provided in an accessible way.
  • Actively listens and receives information, gives, and acts on feedback. Understands the communication needs of others and adapts communication style accordingly.
