FULL DESCRIPTION

Band 6 Specialist Practitioner - Devizes

The postholder will be an autonomous practitioner, able to evidence sound clinical judgement and clear clinical rationale in decision-making. The role involves providing initial mental health assessments and supportive conversations for people who may require access to secondary mental health services.

Main duties of the job

  • Undertake routine and urgent mental health assessments, including triage.
  • Work flexibly across pathways, responding to fluctuating demand.
  • Use creative and solution-focused approach to problem-solving.
  • Coordinate appropriate onward care and support.
  • Maintain accurate, timely, and high-quality documentation.

Job responsibilities

  • Contribute to delivering the primary care mental health service model.
  • Be point of contact for patients from primary care.
  • Work closely with primary care team, social prescribers, recovery navigators.
  • Facilitate mental health and strengths-based assessments.
  • Build links between PCNs and CMHTs, Social Care, community services.
  • Provide formal and informal learning opportunities for primary care staff.
  • Train and supervise junior staff where required.
  • Promote positive risk-taking and safeguarding.
  • Maintain health, safety and security.
  • Participate in clinical audit and service evaluation.
