Job Summary

Are you an experienced clinical pharmacist with proven leadership qualities who is passionate about advancing clinical pharmacy services in primary care? The [Employer hidden — sign up to reveal] is looking for 2 Senior Clinical Pharmacists to join their growing pharmacy team, to support and enhance the clinical pharmacy service at practice level. This is an exciting opportunity, in a new and innovative supernumerary post, to collaborate with an experienced lead pharmacist, as part of an established and innovative pharmacy leadership group, developing the knowledge and skills of our team and the clinical and operational services it delivers, to meet the diverse needs of our patients.

Main Duties

  • Patient facing medicines support and medication reviews
  • Medicines related clinical support for care homes
  • Long-term condition clinics (e.g., respiratory, cardiovascular)
  • Domiciliary structured medication reviews
  • Risk stratification using clinical system searches
  • Management of medicines at discharge from hospital
  • Repeat prescribing and reauthorisation
  • Service development and quality improvement
  • Education, mentoring, and clinical supervision

Qualifications & Experience

Essential: Registration with General Pharmaceutical Council, Independent Prescribing qualification, minimum 2 years post-qualification in General Practice or 5 years in other clinical pharmacy sectors. Desirable: Clinical Diploma, membership of PCPA or RPS.
