PCN – Clinical Pharmacist

[Employer hidden — sign up to reveal] is seeking a Clinical Pharmacist to join their growing Pharmacy Team based at Chatsworth Road Medical Practice, Chesterfield. This permanent, full-time role offers a salary of £48,035.66 to £53,390 a year with NHS pension and benefits.

Job Summary

Are you a pharmacist passionate about developing your skills in patient clinical care? The Chesterfield and Dronfield PCN is looking for a Clinical Pharmacist to work alongside a team of Pharmacy Technicians at Chatsworth Road Medical Practice. Main Duties

  • Patient-facing Structured Medication Reviews
  • Management of long-term conditions
  • Medicines optimisation at transfer of care
  • Repeat prescription authorisations and reauthorisations
  • Acute prescription requests
  • Collaboration with practice and PCN teams to improve health outcomes

It is part of The Chesterfield and Dronfield PCN, one of the largest Primary Care Networks in the UK, serving over 100,000 patients. The role is embedded in one practice but part of a larger PCN pharmacy team.

Person Specification

Skills & Knowledge (Essential)

  • Awareness of common acute and long-term conditions
  • Excellent interpersonal and negotiating skills
  • Excellent verbal & written communication skills
  • Ability to communicate complex information to patients
  • IT systems proficiency (clinical systems, word processing, spreadsheets)
  • Ability to obtain and analyse technical information
  • Work under pressure and meet deadlines
  • Work independently and as a team member

Qualifications (Essential)

  • Mandatory registration with the General Pharmaceutical Council
  • Understanding of relevant standards of practice

Other Requirements

  • Adaptable and self-motivated
  • Meets DBS standards
  • Access to own transport and ability to travel across locality
