Job Introduction
This is the role for you if you want to make a real difference to people’s recovery, independence and wellbeing after a stay in hospital or a period of ill health. Our Home First Reablement Team supports hospital and community referrals for recovery and rehabilitation for up to six weeks. We work at pace to help people regain skills, confidence and independence in their own homes, keeping the flow between hospital and community moving and reducing the need for long‑term care packages. You’ll be part of a dedicated, therapy‑led service that focuses on enablement rather than dependency, supporting single‑handed optimisation of care wherever possible. Main Responsibility
- Deliver high‑quality recovery and rehabilitation support to people with low, medium and complex needs following hospital discharge, injury or ill health.
- Support people using person‑centred, empowering approaches that promote independence, confidence and daily living skills.
- Follow individual care and rehabilitation plans, responding to changes in need and escalating concerns appropriately.
- Record information accurately using council systems, including electronic duty notes, work logs and medication administration records.
- Work safely at all times, following safeguarding, infection control, moving and handling and health and safety guidance.
- Provide personal and intimate care where required, always maintaining dignity and respect.
The Ideal Candidate
- An understanding of the needs of vulnerable adults and a positive, enabling approach to care and recovery.
- The ability to work with people with a range of needs, including dementia, complex health conditions and mobility challenges.
- Clear and confident communication skills, including accurate written recording and effective verbal handovers.
- An understanding of confidentiality, safeguarding and risk management in frontline care settings.
- The confidence to respond calmly in complex or changing situations, knowing when to seek guidance.
- Access to your own transport to travel across [Employer hidden — sign up to reveal] and surrounding areas.
Full UK Driving Licence and access to a vehicle
To be successful in this role, you’ll need a full UK driving licence and access to a vehicle. If we offer you the role, we’ll ask you to provide evidence of your driving licence and vehicle insurance, including cover for business use. We’re unable to confirm an appointment until these documents are received.
As part of the pre-employment checks, should you be offered the role, we’ll carry out an Enhanced Disclosure and Barring Service (DBS) check, (unless you are currently registered with the DBS Update Service).

Our roles are either designated ‘site based’ or ‘home based’. ‘Site based’ roles can include up to two days a week working from home whilst ‘home based’ roles are based at home four days a week. Of course, managers designate which category roles are in based on the needs of the service and the circumstances.
