The role involves managing staff schedules, liaising with clients and families, ensuring CQC compliance, and supporting the care team. Requires previous experience in care coordination or similar, strong organisational and IT skills, NVQ Level 3 in Health & Social Care, and a valid UK driving licence.
Role Overview
The Care Coordinator plays a vital role in ensuring the smooth delivery of care services. This includes managing staff schedules, liaising with service users and families, maintaining compliance with regulations, and supporting the care team.
- Care Planning & Coordination: Develop and maintain individual care plans, allocate care staff, adjust schedules.
- Staff Management & Support: Supervise staff, conduct performance reviews, identify training needs.
- Client & Family Liaison: Serve as point of contact, address concerns and feedback.
- Compliance & Safeguarding: Ensure CQC regulation compliance, maintain records, uphold safeguarding.
- Administrative & Operational Support: Maintain records, coordinate recruitment and training.
Requirements
- Previous experience in a Care Coordinator role or similar in domiciliary care or supported living.
- Strong understanding of CQC regulations and compliance.
- Excellent organisational and problem-solving skills.
- Strong IT skills, including experience with care management systems.
- NVQ Level 3 in Health & Social Care (or willingness to work towards it).
- Valid UK driving licence and access to a vehicle.
