Children's Palliative Community Nurse
[Employer hidden — sign up to reveal] is seeking an experienced Community Nurse to join our Three Counties Hospice in Worcester. This part-time role (22.5 hours/week) delivers specialist palliative care to children and young people with complex health needs.
About the Role
At [Employer hidden — sign up to reveal], we provide specialist palliative care for children and young people with life-limiting and life-threatening conditions, supporting families through every step. Our Children's Palliative Community Nurses are part of a supportive, multi-disciplinary team delivering tailored care.
- Deliver evidence-based end of life and symptom management and after death care in community settings.
- Use clinical skills including long term ventilation, tracheostomy care, oxygen therapy, suction, syringe drivers, and enteral feeds (competency-based training provided).
- Lead assessment, development and evaluation of nursing care plans for children with complex health needs.
- Empower children, young people and families to make decisions about care.
- Travel within a designated geographic area for home visits.
- Autonomous lone working with clinical decision-making and coordination with other healthcare providers.
About You
- Registered Children’s nurse, completed preceptorship
- Demonstrable relevant experience in palliative care
- Proven experience of delivering care within a community environment
- Experience of working with children with complex health needs
- Ability to travel independently around the [Employer hidden — sign up to reveal] region
