Contract: Permanent, 37.5 hours per week

Salary: £38,298.33 – £46,117.70 plus London allowance (depending on experience)

Job Summary

At [Employer hidden — sign up to reveal], we are looking for a Palliative Care Nurse willing to develop into a Clinical Nurse Specialist. The Community Palliative Care Nurse acts as a keyworker for people living with cancer and other life-limiting illnesses. They conduct holistic needs assessments, develop comprehensive care and support plans, deliver high standards of personalised care, and coordinate services.

Key Responsibilities

  • Act as keyworker for patients with life-limiting illnesses
  • Hold regular supported conversations and holistic needs assessments
  • Develop and implement comprehensive care and support plans
  • Deliver physical and emotional support to patients
  • Coordinate care services and inform patients on clinical and practical issues
  • Work as part of a 7-day service with an 8am-8pm rota
