Project Manager - Young Adult Services

[Employer hidden — sign up to reveal] is seeking an experienced Project Manager to lead the transformational development and delivery of a transition service for young adults with life limiting conditions.

Band 7 £41,659 - £47,672 per annum pro rata | 30 hours/4 days per week (full-time considered) | 2 year fixed term contract

About the Role

The Project Manager for young adult services will work proactively with key stakeholders, including young adults, their families and relevant health and social care providers, to design, develop, deliver and evaluate a Young Adults Transition Service. The post-holder will have excellent engagement skills and will promote and champion the needs of young people with life limiting conditions through the development of transition pathways and services which promote autonomy and independence.

What You Need

  • Degree or relevant experience in a specialist area and/or evidence of post qualifying/degree and continuing professional development
  • Experience in successfully managing and delivering change
  • Passion for enabling young adults to have equitable access to specialist palliative and end of life care
  • Enthusiastic, flexible, and a great team player
