Salary: £34,203 - £37,067 per annum (Pro rata)

About the role

We are looking for an Adult Social Care Worker who is passionate about ensuring people have choice and control over their own support needs and focus on the person, their strengths and networks.
The team works with individuals who experience a diagnosed learning disability to understand their social care needs and look at what support can be provided to increase independence and meet identified outcomes. The team responds to safeguarding concerns and undertakes enquiries. Reviews are completed for people in the community in receipt of social care support and people who reside in residential care.

What will you be doing?
We have an opportunity for a 6-12 month fixed term contract for an Adult Social Care Worker in the Learning Disability or within our Flow & Capacity teams.
- Act as 'trusted assessors' for defined areas of support plans.
- Conduct or support individual assessments and reviews of support plans (including risk assessments of both the customer and carer, in line with eligibility criteria to assess the dependency needs of the customer) or facilitate self-assessments, using the appropriate assessment tools and techniques.
- Establish clear, relevant, proportionate, cost effective and appropriate options for the individual.
About you

- Have the passion to promote people’s strengths and the ability to make a difference in a safe working approach to social work practices.
- You are proactive and can think creatively. You demonstrate problem solving abilities, working alongside people to enable them to find their own solutions.
- Have some knowledge of legislation and demonstrable experience of delivering social care outcomes to people in need.
- Able to prioritise, using a risk-based approach.
- You put your ability to analyse and take action in challenging situations to good use and are confident in seeking support.
- You are willing and able to work collaboratively with a wide range of partners, including community groups and the voluntary sector as well as colleagues in health and other sectors.
- You love a challenge and have an openness to learn.
- Have experience working with Adults with Learning Disabilities.
- You will be required to drive and have transport.
Bring your experience supporting adults through strength‑based approaches and working with multi‑agency teams. If you’re confident with IT and hold a degree or NVQ Level 4 (or equivalent knowledge), plus GCSEs at grade C or above, we’d love to hear from you.
