About the role
Our Reablement Support Officers work with a number of people in the community each day. You’ll travel to the people’s homes to support them with regaining skills and confidence to enable them to become more independent, actively promoting their choice and dignity. It’s an incredibly rewarding role, every day and every visit are different! You’ll work with autonomy and independence but within a close-knit team, and you’ll have regular support from the Reablement Team Manager. You’ll also work closely with other our health partners such as the intermediate care team.
Shift Pattern
- Cycle: 4 days on, followed by 4 days off
- Shift Breakdown (per 4-day block): 2 Morning Shifts, 2 Evening Shifts
- Total Weekly Hours: 16 or 31.5 hours
- Shift Times (16hrs): Morning 07:00-11:30, Evening 18:00-22:30
- Shift Times (31.5hrs): Morning 07:00-16:00, Evening 13:30-22:30
- Includes weekend and bank holiday working
What will you be doing?
- Visit individuals in their own homes, working independently or occasionally with a colleague.
- Monitor progress and support with day-to-day activities including personal care, meal preparation, mobility and accessing the community.
- Case manage Reablement journeys from initial assessment through to discharge.
- Assess needs after referral and support individuals in planning their Reablement goals.
- Signpost to other agencies such as Assistive Technology, Age UK, Northamptonshire Carers.
About you
- Confident working independently with ability to handle unexpected situations and solve problems.
- Previous experience within a health or social care setting.
- Either currently have or be willing to work towards NVQ level 2 in Health & Social Care or equivalent.
- Full driving licence and access to own vehicle required (travel countywide within Northamptonshire).
- This role is subject to an enhanced DBS certificate (cost covered by [Employer hidden — sign up to reveal]).

- £14.88 - £15.68 per hour, depending on experience.
