Practice Nurse

Practice: [Employer hidden — sign up to reveal] is situated in Saxilby, Lincolnshire with a list size of 8,200 patients. It is a GMS, EMIS web Practice.

Job Plan: To support patients in improving mental, physical, and behavioural health through health checks, chronic disease management (asthma, diabetes, COPD), immunisations, cervical cytology, wound care, and health promotion. Also includes infection control and participation in QOF/DES requirements.

Qualifications: NMC-registered with recent primary care experience, confident in managing long-term conditions, skilled in EMIS (desirable), compassionate, organised, team player.
