Job summary

Join our friendly, forward-thinking GP practice as a Health Care Assistant (HCA) and play a vital role in delivering high-quality, patient-centred care to our local community. You'll support our clinical team with a wide range of duties including NHS Health Checks, long-term condition monitoring, and health promotion, helping patients improve their health and wellbeing every day.

Main duties of the job

  • Perform routine clinical observations including blood pressure monitoring, height, weight, BMI, ECG recordings, urinalysis, and phlebotomy (if trained).
  • Undertake NHS Health Checks and support chronic disease monitoring.
  • Conduct home visits for housebound patients.
  • Support immunisation and vaccination programmes.
  • Carry out new patient health checks and provide basic health promotion advice.

Job responsibilities

  • Perform clinical observations: blood pressure, height/weight/BMI, ECG, urinalysis, phlebotomy.
  • NHS Health Checks and chronic disease monitoring (diabetes, hypertension, COPD).
  • Home visits for housebound patients.
  • Maintain accurate records using EMIS Web and SNOMED coding.
  • Manage stock levels and clinical equipment.
  • Safeguarding responsibilities: recognise and escalate concerns.
  • Adhere to infection prevention and control policies.
  • Participate in training, appraisal, and service development.

Person Specification

Essential

  • NVQ Level 2 or 3 in Health and Social Care or equivalent experience in Primary Care.
  • Care Certificate (or willingness to complete).
  • Experience in healthcare setting and direct patient care.
  • Knowledge of basic clinical procedures and infection control.
  • Ability to use EMIS Web.
  • Strong communication and teamwork skills.

Desirable

  • Phlebotomy certification, ECG training, NHS Health Check training.
  • Experience in General Practice and supporting long-term conditions.
  • Knowledge of SNOMED coding and QOF frameworks.
