Job summary
Join our friendly, forward-thinking GP practice as a Health Care Assistant (HCA) and play a vital role in delivering high-quality, patient-centred care to our local community. You'll support our clinical team with a wide range of duties including NHS Health Checks, long-term condition monitoring, and health promotion, helping patients improve their health and wellbeing every day.
Main duties of the job
- Perform routine clinical observations including blood pressure monitoring, height, weight, BMI, ECG recordings, urinalysis, and phlebotomy (if trained).
- Undertake NHS Health Checks and support chronic disease monitoring.
- Conduct home visits for housebound patients.
- Support immunisation and vaccination programmes.
- Carry out new patient health checks and provide basic health promotion advice.

Job responsibilities
- Perform clinical observations: blood pressure, height/weight/BMI, ECG, urinalysis, phlebotomy.
- NHS Health Checks and chronic disease monitoring (diabetes, hypertension, COPD).
- Home visits for housebound patients.
- Maintain accurate records using EMIS Web and SNOMED coding.
- Manage stock levels and clinical equipment.
- Safeguarding responsibilities: recognise and escalate concerns.
- Adhere to infection prevention and control policies.
- Participate in training, appraisal, and service development.
Person Specification
Essential
- NVQ Level 2 or 3 in Health and Social Care or equivalent experience in Primary Care.
- Care Certificate (or willingness to complete).
- Experience in healthcare setting and direct patient care.
- Knowledge of basic clinical procedures and infection control.
- Ability to use EMIS Web.
- Strong communication and teamwork skills.
Desirable
- Phlebotomy certification, ECG training, NHS Health Check training.
- Experience in General Practice and supporting long-term conditions.
- Knowledge of SNOMED coding and QOF frameworks.
