Main duties of the job
The Home Treatment Team (HTT) provides a 7 day a week service, between the hours of 8am and 10pm, alongside a twilight shift which is 5.30pm until 3am for gatekeeping. Key responsibilities include assessing and delivering care for people in crisis. The team is multi-disciplinary with a strong ethos of team working and support. The post holder will work on a shift basis over a 7 day period. The HTT works closely within the crisis network. The Band 6 social worker will provide clinical care, supervise junior staff, and participate in team development.

Job responsibilities
Shift patterns: Days 08:00-18:00, Late 12:00-22:00. Job purpose includes comprehensive assessment of new referrals, contributing to crisis system, rotating onto shift coordinator, carrying out initial HTT visits, linking to other clinical areas, delivering supervision, and contributing to service development. General requirements: maintain Social Work England registration. Principal duties cover clinical triage, assessment, risk management, care planning, evidence-based interventions, collaboration with service users and agencies, maintaining records, leading on MCA/DOLS, MHA proceedings, CTOs, 117 aftercare, social care assessments, health interventions (WRAP, psychosocial, CBT, DBT), medication management, liaison with MARAC/MAPPA, guardianship. Supervision includes regular clinical supervision, delivering supervision and PDR for junior staff, participating in own PDR and CPD. Service development and education/training also required.

Qualifications (Essential)
- Evidence of continued professional development
- Registered Social Worker in Mental Health
- Experience of working as a Band 5 or 6 Mental Health Social Worker in a community and MDT setting
Knowledge & Skills (Essential)
- Working knowledge of Care Programme Approach & Care Management
- Knowledge of appropriate therapeutic approaches
- Knowledge of PDR and supervision processes
- Demonstrable risk assessment and risk management skills
- Demonstrable caseload and time management skills
- Ability to work autonomously and as part of a multi-disciplinary team
- Ability to assess, plan and evaluate care
Experience (Essential)
- Experience of multi-disciplinary team working
- Experience of working in a community setting
- Experience of mentoring others
- Excellent communication skills both verbally and written
